ReliantHealth Primary & Wellness Care

Why Choose ReliantHealth?

Our Commitment to You

ReliantHealth commits to providing accessibility, comprehensiveness, coordination, and continuity to your primary care by prioritizing patient needs above all else, and promoting evidence-based medicine alongside compassionate communication and advocacy.

Our History

ReliantHealth was established in 2021 as a division of Reliant Physicians health group. Our clinic was initially built around the needs of our recently discharged hospital patients who did not have a board-certified primary care doctor to care for them after discharge. We have now evolved to offer comprehensive primary care and wellness coaching for overall optimal health.

Our Values

ReliantHealth values relationships – with our doctors, clinicians and staff, with our growing community, with our specialist partners, and above all with you, our patients. We provide an inclusive, equitable and effective care environment for our patients and their families.

Reliant's
Best Care Model

We believe in a modern, multi-modal approach to your health. Our clinics focus on high value, directed care than benefits you as a person as well as a patient. Some aspects of optimal primary care that we practice:

Risk-stratified care

This involves identifying patients who are at high risk of developing chronic conditions and providing them with targeted interventions to prevent or manage these conditions.

Care coordination

We make sure our patients receive the right care at the right time by coordinating care across clinical settings and with all our providers.

Careful Selection of Specialists

We focus on finding the very best specialists for your needs and vet them for quality and clinical performance.

Patient-centered care

This involves engaging patients in their care by involving them in decision-making and providing them with the information and support they need to manage their health.

Team-based care

This involves building capacity and skills of primary care staff and clinicians to work together effectively to provide high-quality care.
